Enrollment Requirements

 

Age Requirements
All students entering Kindergarten must by five (5) years old on or before August 31, 2008. First grade students must be six (6) years old on or before August 31, 2008 - or have attended an accredited Kindergarten, according to Kansas Statute K.S.A. 72-1107 which defines the entrance requirements for the state.

New Students
Parents of students entering kindergarten and students new to the St. Xavier School should bring the following items with them to enroll:

  • Proof of Identity - A birth certificate is required for Kindergarten.  If this is unavailable, schools will accept a visa or passport.  Students may enter kindergarten if they will be five years old on or before Aug. 31, 2008. Students may enroll in first grade if they will be six years old on or before Aug. 31, 2008.

  • Certified Transcripts or Grade Cards

  • Kansas Health Assessment - Students up to age nine who have never attended a Kansas school should present the results of a health assessment upon entering school. Students will not be excluded from attending school if they do not have the assessment, but they are strongly encouraged to complete this form. To comply with this law, 1992 KSA 72-5214, the assessment:

    • Must be conducted by a physician, a person working under the direct supervision of a physician and cosigned by that physician or a nurse who has special certification from the state to perform these assessments.

    • May be done up to 12 months prior to school entry or up to 90 days after school entry.

    • May be recorded on any health assessment form that includes health history, physical exam and screening tests as medically indicated by the provider.

    • Applies only to students age eight and under who have never been enrolled in any Kansas school.

  • Immunization Requirements - Before students may enter a Kansas school, an immunization certificate (officially referred to as a Kansas Certificate of Immunization or KCI), signed by a physician or health department representative, must be provided listing the month, day and year that vaccines were given. Some legal alternatives are available.
